Commentary (Moller): Surgical Staging in Endometrial Cancer

نویسنده

  • A. Moller
چکیده

Endometrial cancer is the most common gynecologic malignancy affecting women in the United States. In 1988, the International Federation of Gynecology and Obstetrics shifted from a clinical staging protocol to one based on surgical factors, making surgical staging the accepted treatment approach to endometrial cancers, with excellent survival compared to other gynecologic malignancies. The manuscript by Kirby et al brings to light the controversies surrounding the surgical evaluation of endometrial cancers. Although surgical staging has been shown to have both prognostic and therapeutic benefit, major problems in the United States continue to result in suboptimal treatment of patients with endometrial cancer. These problems include the lack of an accepted surgical protocol (in terms of adequacy of lymph node sampling) and incomplete surgical staging secondary to patient factors or the lack of referral to specialty-trained gynecologic oncologists.

مقایسه یافته‌های مشاهده مستقیم و پاتولوژی حین عمل با پاتولوژی نهایی در سرطان اندومتر

Background: Endometrial carcinoma is considered the most common gynecological cancer in the world. Pelvic and para-aortic lymphadenectomy is widely advised based on FIGO staging system. Intra-operative frozen sections analysis is used to identify pa-tients at high risk for pelvic and para-aortic nodal metastasis evading lymphadenec-tomy in low-risk patients. However there is still some controve...

Comprehensive surgical staging for endometrial cancer.

Despite a shift from clinical to surgical staging of endometrial cancer in 1988, performance of comprehensive surgical staging for clinically early-stage endometrial cancer remains controversial. Low-, intermediate-, and high-risk groups have been defined pathologically. Herein, we describe the risks and benefits of comprehensive surgical staging. Comprehensive surgical staging is encouraged in...

Sentinel lymph node detection in endometrial cancer: hysteroscopic peritumoral versus cervical injection.

We read with great interest the article by Bogani et al. [1], highlighting some of the debated issues regarding the role of sentinel lymph node (SLN) mapping in endometrial cancer. However, taking a cue from this commentary and from the available evidence in the literature regarding the surgical staging of endometrial cancer, we would like to broaden the debate to include several important aspe...

The role of comprehensive surgical staging in patients with endometrial cancer.

BACKGROUND The cornerstone of the management of patients with endometrial cancer is hysterectomy. Since 1988, the role of lymphadenectomy for patients with endometrial cancer has been debated. Patients who undergo pelvic and para-aortic lymphadenectomy are more likely to be accurately staged and are less likely to receive adjuvant radiation therapy. METHODS The authors perform a narrative rev...
